Is This Purging or Breaking Out? The Science to Tell the Difference

In Plain English
Skin purging is a temporary increase in breakouts when starting a new active that accelerates cell turnover — retinoids, AHAs, BHAs, and vitamin C. The active brings pre-existing congestion to the surface faster than normal. It looks like a breakout but it is actually the clearance phase. It resolves within 4-8 weeks and leaves skin clearer than before. A true breakout from a product reaction does not resolve — it persists or worsens, and appears in new areas where you do not normally break out.

The Science of Skin Purging

The skin has a natural cell turnover cycle of approximately 28 days (slowing to 45-60+ days with age). Microcomedones — the precursors to all visible breakouts — form 2-3 weeks before they become visible as papules, pustules, or open comedones. When a cell-turnover-accelerating active (retinoid, AHA, BHA) is introduced, this cycle speeds up, pushing existing microcomedones to the surface faster and simultaneously. The result is a cluster of breakouts appearing in the first 2-6 weeks that represents the pre-existing congestion queue being rapidly cleared, not new congestion being created. (Leyden JJ et al. PMID: 7333297)

Purging vs Breakout: The Diagnostic Framework

Signs it IS purging:
— Started within 2-4 weeks of introducing a cell-turnover active (retinoid, AHA, BHA, vitamin C)
— Breakouts appearing in your usual breakout zones (not new areas)
— Breakouts are smaller and faster-cycling than your normal breakouts — come and go quickly
— Overall skin texture improving even as individual breakouts appear
— Resolving by week 6-8 with progressive improvement

Signs it is NOT purging (product reaction / breakout):
— Breakouts appearing in new areas where you do not normally break out
— Started with a product that does not accelerate cell turnover (moisturiser, SPF, serum without actives)
— Breakouts are large, cystic, or fungal-type (small itchy clusters) rather than the usual papules/pustules
— Not resolving or worsening after 8 weeks
— Accompanied by redness, burning, itching, or rash — this is irritation or allergic contact dermatitis, not purging

Which Actives Cause Purging

Purging is expected from: Retinol, retinaldehyde, tretinoin, adapalene (all retinoids) — the most common purging trigger. AHAs (glycolic acid, lactic acid, mandelic acid) — accelerate desquamation and surface congestion clearance. BHAs (salicylic acid) — clears follicular congestion which surfaces as temporary breakouts. Vitamin C (L-ascorbic acid) — mild cell turnover acceleration in some individuals.
Purging is NOT expected from: Moisturisers, SPFs, hyaluronic acid, niacinamide, peptides, ceramides. If you break out from these, it is a product reaction, not purging — stop using them.

The SS Skin Purging Protocol

During purging — what to do:
1. Continue the active that is causing purging — stopping resets the clock and delays the clearance phase
2. Reduce frequency if purging is severe: retinol 3x/week instead of nightly; AHA 1x/week instead of 3x
3. Strengthen the barrier to reduce irritation: Hyaluron 3D Hydrator and ceramide moisturiser
4. Add Black Rice Niacinamide Concentrate — anti-inflammatory, reduces purge severity without interfering with cell turnover
5. Do not add more actives during purging — isolate the cause
6. Do not over-cleanse or over-exfoliate to ‘help’ the purge — this worsens barrier disruption and prolongs it

AM during purge: Gentle cleanser — HA serum — Niacinamide 10% — lightweight ceramide moisturiser — mineral SPF 50+

PM during purge: Gentle cleanser — HA serum — active (retinol/AHA/BHA at reduced frequency) — Peptide Serum — ceramide moisturiser

Do not: Pick or extract purging breakouts (causes scarring and PIH). Add spot treatments with harsh actives (BP, high-strength acids) on top of purging skin. Stop the active prematurely (6-8 weeks minimum before judging).

Shop This Protocol

Black Rice Niacinamide Concentrate — Anti-inflammatory support to reduce purge severity

Peptide Serum with Custard Apple and Blood Orange — Gentle collagen support that does not interfere with purging actives

Results Timeline

Week 1-3: Purging begins — increased breakouts in usual zones, faster cycling.
Week 4-6: Purge peaks and begins to resolve. Skin texture improving.
Week 6-8: Purge resolved. Skin visibly clearer than pre-active baseline.
Month 3+: Full benefit of the active realised with continued use.
